Case 10 Expired; see diagnosis below. This 45 year old male patient presented with epigastric distress with radiation to the upper lumbar region. He was hypotensive, hypovolemic, and in respiratory distress. There was abdominal tenderness noted. What is your diagnosis and what labs would you like to see drawn to help differentiate the cause of his symptoms and this ECG? Post your answer in the space provided below and submit. If you are correct your name will be posted as the winner along with the answer, and a new case will be posted.
Answer to Case study number 10 above. No one got the answer right. Everyone who responded called it an acute myocardial infarction due to the ST elevation in V2-V4. This patient has acute pancreatitis and it is not uncommon for it to mimic an acute infarction. Beware: all that is up is not infarction.
Can you imagine making this mistake and giving this patient a thrombolytic agent in the hospital where PCI is not available? We would have caused a real hemorrhage and death of the patient.
